What to Bring

The needs for patients vary for both inpatient and outpatient treatment.

If you or a loved one are coming to ACI for outpatient services, you should bring your insurance card, a photo identification, and referral paperwork (if available).

For inpatient services, you should bring your insurance information, a photo identification, referral paperwork (if available), two to three days worth of clothing and undergarments, and any medications you are currently taking.

“I firmly believe that ACI is why my son is still alive, back with our family, and back to school. As a mother my primary concern was that my son would receive compassionate care and the attention he needed to focus on his recovery.
